How to use the consent form dashboard

Navigate to the Consent form dashboard by selecting it on the left panel selector.

 

 

You can view any submitted form for a particular form by clicking on the submissions section of that form.

 

You can also view submission via PATIENT or FORM by toggling between the two.

 

 

 

It will also highlight the row pink to notify you, you have a new consent or intake form to review.

You can mark as read or unread.

 

The completed form can be viewed here and also on the individual patient dashboard under All Submissions.
