How to create my own template

Go to settings and then clinician forms.

Here you can use the add new button to upload your template images and use the template builder to create you form notes, using fields such as text field, yes/no. number fields, images, multiple choice and spreadsheets.

Name and save the template to then be able to select it to use for creating a form note.
