Introduction on creating a consent form

To create a consent or intake form, select the consent form dashboard icon on the left hand side.

It will display any consent forms you may have.

 

 

You can navigate to the template library to download to your account some consent forms in the template library.

 

 

Otherwise you can create a new consent form using the template provider by CREATE Form.

 

 

This will open the consent form builder.

 

Here you can name a your consent / intake form, choose to allow registration to create it as an intake registration form and then select ADD FIELD to create fields and utilize the form builder.
